About the Role

The Intensive Intervention Services provides intensive and time-limited crisis-stabilization interventions and supports to individuals with mental illness and substance abuse disorders. This stabilization program is intended to respond to Allegany County residents who are experiencing acute symptoms or crisis. Intensive Intervention Services assists adults in obtaining needed medical, social, recreational, educational, financial, vocational and other services through assessment, planning, coordinating, securing and monitoring services.

The assigned program is funded by the New York State Office of Mental Health (OMH) as part of reinvestment dollars and is required to adhere to all OMH regulations and guidelines and the US Department of Health and Human Services.

Duties:

Serve as Primary Coordinator:

Review the SPOA application, supporting documentation and consult with the SPOA coordinator.
Contact the referral source for further information, as needed
Within 24 hours contacts the individual to schedule an initial face-to-face contact in their home or community location.
Refer to or link client to other community-based service providers as an alternative to inpatient evaluations and/or hospitalization, as indicated by the family and needs assessment.
Obtain releases of information for all services that the individual is involved with.
Complete a rehabilitation assessment that will identify client needs, risks, supports, cultural information, and religious information. This will provide the groundwork for work with the individual.
Responsible for gathering collateral input into the assessment, action plan and WRAP plan.
Develop an individualized service plan with the client and implement within 14 days of referral to the program. The Service Planning process is recovery-oriented, building upon the client’s strengths to help meet his/her needs.
Conduct a Service Plan Review at 4 weeks of service in order to monitor the client’s progress toward treatment goals and to determine the continued need for service provision. It is anticipated that, through specialized support and linkages, the client will have achieved a reasonable stable level of functioning within 6 -10 weeks.
Responsible for providing or making arrangements for necessary medical, dental, mental health, financial, legal, social, educational, psychological and/or other assessments, as needed for the client and/or family as identified in the assessment process. Utilize WRAP around funds to address needs as appropriate.
Discuss relevant cultural, religious, ethnic, racial, developmental, age, social, sexual orientation, geographic, and other factors that influence and play a role in the client’s life.
For clients needing services beyond 6-week period, submit an Extension Request to the SPOA coordinator.
Be aware of and adhere to HIPAA regulations and agency’s confidentiality and record keeping policies and procedures.
Coordinator will be placed on the crisis hotline rotation schedule to provide crisis coverage.
Perform all necessary discharge and termination functions for individual clients as needed within one month of discharge/termination.
Maintain contact with other service providers based on individual needs of client and family.
Must participate in clinical team treatment environment and adhere to established clinical rehabilitation theories and practices unless ethical conflict occurs.
Report any ethical conflict to supervisor or director.
Benchmarks/Time Management

Within 24 hours of receiving the referral contact the individual to schedule an appointment.
Within 7 days of referral complete rehabilitation assessment through face-to-face meetings with client, family members, and collateral contacts.
Complete the initial Columbia suicide assessment with the person the first week and complete a since last visit assessment at each subsequent week.
Within the first 2 weeks of services, maintain 3 contacts each week either face to face or through phone and at least 2 weekly contacts thereafter. The contacts occur at home or in the community as needed by the client. Contacts may exceed the required frequency and are expected to last as long as necessary to provide the appropriate services and interventions needed
Clinical record documentation will identify the individual’s short term and long-term goals, strengths, interests, and needs.
Maintain a caseload capacity, averaging 4-6 adults. In an exempt employment status, the clinician is expected to complete assigned work within the timeframes specifically requested or expected regardless of the work week schedule.
Recording

Complete all clinical documentation for assigned caseload in the timeframe as outlined by program policies and practices and/or the NYS OMH regulations.
Assure documentation and records comply with the NYS OMH. Make corrections as necessary in documentation style or counseling techniques.
Be familiar with and adhere to the ARA Personnel Policy and its Corporate Compliance Plan.
Provide caseload program stats and reports monthly.
Complete incident reports as needed per NIMRS and Justice Center guidelines.
Supervision

Participate in regular supervision sessions with the assigned supervisor.
Complete work assignments made by the supervisor in a timely fashion.
Participate in assigned clinical and non-clinical training of mandatory and non-mandatory status as provided or approved by agency.
Conduct professional self-evaluation and participate in evaluation interviews with assigned supervisor.
Participate with assigned supervisor and director implementing performance improvement plans and the progressive disciplinary action policy
Committees

Serve on committees as assigned by supervisor.
Transportation of Clients: At times a staff member may be requested to transport clients in agency vehicle to community activities, resources and agencies. Agency vehicles will be utilized when available. It is therefore necessary for employees to possess a valid NYS license or other valid state driver’s license.

Minimum Requirements:

Master's degree in social work, counseling, psychology or related field with two years’ experience in mental health or other human services, working with adults is preferred.
Bachelor's degree in social work, counseling, psychology or related field with a minimum of four years’ experience in mental health or other human services, working with adults is acceptable.
Strong computer abilities and proficiency with electronic medical records (EMR).
Professional appearance and demeanor.
Valid background clearances with NYS Justice Center and Office of Child and Family Services.
Valid driver’s license.
